UNIVERSAL PRIMARY CARE AWARDS

Yearly HRSA provides Quality Improvement Awards (QIA) to Community Health Centers. These awards recognize the highest performing health centers nationwide, as well as those health centers that have made significant quality improvement gains from the previous year. The award monies support HRSA’s strategic goal to improve access to quality health care, and directly supports the U.S. Department of Health and Human Service’s goal of moving towards a value-based payment system by improving the quality, efficiency, and overall value of primary health care delivered by health centers to our patient.

This year UPC received the following 2018 HRSA Health Center Quality Improvement Grant Awards:

  1. The Health Center Quality Leaders Award – UPC was recognized for achieving the best overall clinical performance among all health centers and placed in the top 30% of the adjusted quartile rankings for clinical quality measures (CQMs).
  2. The Electronic Health Record (EHR) Reporters Award- UPC was recognized for setting up and implementing our EHR to report on all Clinical Quality Measures data for all of our health center’s patients.
  3. The Advancing Health Information Technology (HIT) Quality Award-UPC was recognized for health centers that utilized Health Information Technology systems to increase access to care and advance the quality of care for patients.
  4. Achieving PCMH Recognition– UPC was recognized for health centers who achieve a Patient-Centered Medical Home (PCMH) model recognition in one or more practice sites.

If your local health center is closed, your call will be automatically forwarded to an answering service that will connect you with an on-call provider.  Our telephone lines are open 24 hours a day, seven days a week.  Translation services are available after-hours as well.

Universal Primary Care is a nationally recognized Patient Centered Medical Home (PCMH). This model of primary care has been shown to keep people healthier and improve continuity of care throughout the life cycle. PCHM puts you, the patient, at the center in which you work with our team to create a personalized plan to reach your health care goals.

Our PCMH practice partners with you; and by treating you with respect, dignity and compassion, we build strong trusting long term relationship. We are committed and responsible to help you manage all your health care needs either directly by our providers or by appropriate referral.
